This position supervises and directs the activities of the Healthy Start Federal clinical case management team. The team consists of the Perinatal LCSW and Community Health Workers. In addition oversight over staff contracted with outside agencies including Bay front Resource specialist, Specialty Liaison Supervisor, Clinical Resource Specialist, New Arrival Nurse, and Teen Case Manager. This position directs the case management team, while handling overflow and high risk patients as needed.
Licensed Masters Level Social Work with experience or specialized training in mental health required. Previous supervisory experience preferred. Knowledge of Maternal and Child Health Issues. Knowledge of case management procedures and techniques. Knowledge of quality assurance principles. Knowledge of community resources. Knowledge of methods of compiling, organizing and analyzing data. Knowledgeable of Healthy Start Federal grant requirements and guidelines. Skilled in the use of personal computers, applications and software. Skilled in creative problem solving. Ability to analyze the effectiveness of service programs and identify resources or adjustments needed to meet needs. Ability to plan, organize and coordinate work assignments. Ability to communicate effectively. Ability to establish and maintain effective working relationships with others.
